Please contact us by phone or complete our online request form for an appointment. Our office will schedule your visit as promptly as possible. If you are contacting us with pain or an emergency situation, every attempt will be made to see you that day.
We are committed to meeting the needs of every patient and providing the highest quality of care. While we endeavor to keep your waiting time to a minimum, circumstances may arise that lengthen the time for a particular procedure, or an emergency occurs. We ask for your patience and understanding in this situation.

What to Bring
Prior to your consultation please provide the following information:
- A list of medications you are presently taking
- The referral slips and any x-rays from your dentist
- Any dental insurance card and information
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at each visit.
What to Expect
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Treatment is usually accomplished the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.
